The Care Team - Home Health & Hospice

The Care Team has been delivering clinically-trained home health, hospice, and palliative care to mid-Michigan families for two decades. Our nurses, therapists, and home health aides work as an extension of your loved one's medical team — coordinating with primary-care physicians, neurologists, and the GLCF care navigators to keep care continuous as needs change.

We are Medicare-certified and accept most major insurances. Family caregivers stay informed through a dedicated case manager who is on-call when the home visit isn't enough, and we work with you on the practical, hard moments — medication transitions, hospice conversations, and end-of-life planning — with the same clinical rigor and the same patience.

Partner with GLCF means our intake team prioritizes referrals from the Foundation's care navigators, and we participate in GLCF's quarterly caregiver education series.
